Ahoy there Minecraft users!

I am stuck. You see, in the excitment of Multiplayer Survival, I  wish to change my Skin.
I DO already know how to do this (in paint, a recent make of mine is an ipod (pictured below)) However, I have noticed that some people have things on their skins which I can't seem to do myself.

These are things such as sunglasses or headsets, which are not drawn directly onto the head, but rather they almost pop out. I was wondering if I myself could do such a thing.

Is it possible for me, an average paid player on minecraft?
